You should now complete the online orientation form.  All handouts containing the answers to the questions are posted at the Course Information web page. This orientation form should be completed after the updates for the current semester have been posted, submitted as soon as possible and no later than January 13, 2013, or you may be withdrawn from the course.

Please enter your information for the Online Math Class. When you have finished entering all information, click on "Submit" at the bottom of this form.  All fields are required.

To complete this online orientation, you must answer the following questions. Short answers are fine. The answers to these questions may be found in the course materials, which are all linked to the Home Page for this course.

Do you know your TSI status, and if not, whom should you contact immediately?

What is the name of the textbook for this course?

Where must you purchase a textbook for this course?

Where can you find the license to use the MyMathLab computer program that accompanies your text?

When should you enroll in your section of MyMathLab online, and what is your Course ID? (See the handout Information About MyMathLab - be sure you read the correct one for the course you are taking.)

What should you save, as proof of purchase, when you buy your textbook?

Where may you go (at any of the main ACC campuses) to receive free tutoring for this course?

What is the deadline date for taking the final exam (and finishing this course)?

Comments or Questions?

Select the ACC Campus Testing Center (or OSD Center) in which you will take all five of your exams (4 regular tests and the departmental final) for this course (please remember that if you choose to notify your instructor that you will be taking your tests in an OSD Center, you will need to send her your ACC OSD letter of accommodation as soon as possible and at least 2 weeks before the first test is scheduled to be taken - thanks):
