| Important Information: | ||
| All applications must be received by Austin Community College by July 13, 2011. | ||
| Please send a $25.00 check or money order payable to Austin Community College along with the completed application. | ||
| Each student must bring the completed Medical Information form with them on the first day of the workshop if it was not returned with their application. Students will not be allowed to participate without the signed Medical Information form. | ||
| The $25.00 participation fee will be refunded if the applicant is not accepted. | ||
| Class space is limited and an early response is recommended. | ||
