ECS Forms

Early College Start applicants should visit with their high school counselor to obtain the forms necessary to complete the ECS co-enrollment process. Some forms are downloadable here. Forms Checklist

Forms provided by a high school guidance counselor:

• ACC Admission Application - students use web application process
• Completed ECS Coenrollment and Tuition Voucher
• Out-of-District Tuition Exemption Request
• ECS Non-Citizen Student Data Form ( Estudiantes no ciudadanos sin numero) Note: Non-citizen students without Social Security numbers, who need an alternate number to use for ACC services (i.e. testing, application, registration), must use this form to obtain the number in advance of accessing ACC services. Estudiantes inmigrantes sin número de Seguridad Social quienes necesitan un número alternativo para usar los servicios de ACC (i.e. exámenes, aplicaciones, registro) deben usar esta forma para obtener el número de acceso de servicios ACC por adelantando.
• Application for a COMPASS Fee Waiver
