INFORMATION FOR ALL FACULTY REGARDING INPUT FORMS

The self-evaluation Faculty Input Form is submitted by each faculty member in the Spring semester. This form provides the opportunity to respond in a thoughtful and detailed way to the statistical analysis from students’ evaluations of you and your course, to comments from students, and to document your plans for instructional performance gains based on student evaluations and your own personal plans for improvment in the coming year.

Adjunct faculty failing to submit the Faculty Input Form by the college deadline may be removed from the eligibility list.

The Faculty Input Form. Download the form, enter your responses, attach additional pages if appropriate, save a copy, print a copy, sign it, and send to the Department Chair by the Deadline (Adjunct Faculty: February 14, 2011, Full-time faculty: March 7, 2011).

The evaluation instrument that Faculty Evaluation committee members use to evaluate your Faculty Input Form. Note that there are four required elements: (1) statistical data; (2) student comments; (3) instructional goals based on student evaluations; and, (4) instructional goals based on personal plans for improvement.
