Physical Therapist Assistant
Welcome to the Physical Therapist Assistant Department
To apply to the Physical Therapist Assistant Program, please create an account with Castle Branch (formerly Certified Profile). You will need to use code UL64im and upload your immunizations. Then when you are ready to upload your documents, you will use code UL81im.
Due to conflicting information on the PTA website, the application deadline for 2016 has been extended from March 31st to Monday, May 2nd. All forms that cannot be uploaded to Certified Profile need to be at the Health Sciences office by close of business on May 2nd.
