Congratulations on your admission into the Associate Degree Nursing Program - Hybrid option. In the near future, you will face many nursing challenges. Until then, you face just the following to-do list. It is subject to change and you should check back at least once a month to ensure you have the latest instructions. Criminal Background Check Implementation Process:
- Currently Enrolled and Incoming Students for Spring 2008
- Background Checks may be ordered between between October 1st and December 1st.
- Deadline-December 1st.

Order clinical uniforms: From either TOTAL UNIFORMS locations (7801 N. Lamar #162B (phone # 452-5391 ) or 209 E. Ben White #200A (phone # 441-1525). See "Dress Code" in the ADN Program Student Handbook 2007-2008 for details. Estimated cost for two sets of scrubs, a lab coat and 3 school patches is approximately $100. A deposit of $40.00 is preferred at the time the order is placed. Deadline: December 15, 2007
*You will be given an opportunity to buy second hand scrubs at orientation from the ADNSA. We cannot guarantee that your size will be available.
Purchase the following uniform accessories at any uniform or medical supply store: Deadline: January 1, 2008.
- watch with a second hand
- stethoscope
- pen light
- bandage scissors
- ball point pen (black ink only)
Turn in the following items all together between December 17th and December 20th to the Nursing Office, Room 8227 (ATTN: Sheila or Tammy)
- Copy of CPR card for health care providers (infant, child, adult, two-rescuer, choking)
- Completed "Statement of Understanding." (Located in Student Handbook
- Email address
- Completed health care form, including TB test results done within the last month (signed by primary care provider)
- Confirmation page from St. David’s, Seton, and HIPAA Modules with the signed confidentiality page.
- Seton Workplace Violence Form
- BNE Licensure form

Print the Health Sciences Health Data Form, and take it to your primary health care provider for completion. Deadline to be turned in :

Read the ADN Program Spring 2008 Student Handbook , print and complete the "Statement of Understanding" after December 1st. Deadline: December 20, 2007 (Do not complete until after December 1st)

NEW All students MUST order their own lab pack for use in skills lab. The order link will be available after November 15, 2007.
