START THE TO-DO LIST-See below for submission deadlines:

STEP ONE: Complete Criminal Background Check (CBC) Implementation Process

  • Incoming Students for Fall 2012
  • We can only use CBCs from Pre-Check, we do not accept DPS background checks.
  • Background Checks may be ordered between: May 15-July 15, 2012
  • Deadline – July 15, 2012
  • Click here to be taken to the Background Check page.
  • CBC must come back "Clear" before permission will be given to register.
  • The Nursing Office will send an email when we receive the notification of your clear CBC giving permission to register.

STEP TWO: Obtain a physical and Tb Skin Test using the HEALTH DATA /PHYSICAL EXAM FORM.

  • Download the Health Data Form here.
  • This form must be signed by a physician or a nurse practitioner only.
  • A Tb Skin Test is good for one year. Documentation of a negative two-step TB skin test within the past 90 days prior to beginning the program.
  • Two-Step TB Skin Test requires that the student have one negative TB test, than 1-3 wks later having a second negative TB skin test completed. Please refer to the Health Data form for specific information before obtaining test.
    NOTE: Two-Step Tuberculin Skin Test (TST) Method:
    1st TestNegative:Repeat TST in 1-3 weeks
    2nd TestNegative:Person probably does not have infection
    Positive:Boosted reaction due to infection in the past
  • If you are required to receive a Chest X-ray because of testing positive, please submit a copy of the report. (Chest x-rays are only good for 5 years).

STEP THREE: Order your Lab Pack

NEW All students MUST order their own lab pack for use in skills lab. Please note that part of the kit will be delivered to the students home while the syringes, needles, etc. will be delivered to the nursing school and distributed by the faculty to the student.

If you are taking RNSG 1140 in the Summer 2012, the Order deadline is June 22nd:

If you are taking RNSG 1140 in the Fall 2012, the Order Deadline is August 1, 2012

STEP FOUR: Order clinical uniforms

From either TOTAL UNIFORMS locations: 1301 34th St. (phone 452.5391) or 209 E. Ben White #200A (phone 441.1525). See "Dress Code" in the ADN Program Student Handbook for details. Estimated cost for two sets of scrubs, a lab coat and 3 school patches is approximately $100. A deposit of $40.00 is preferred at the time the order is placed.

Deadline: August 1, 2012.

*You will be given an opportunity to buy second hand scrubs at orientation from the ADNSA. We cannot guarantee that your size will be available.

Purchase the following uniform accessories at any uniform or medical supply store by August 23, 2012:

  • watch with a second hand
  • stethoscope
  • pen light
  • bandage scissors
  • ball point pen (black ink only)

STEP FIVE: CPR Certification- AHA Only

  • Must be American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Providers.
  • The card must say American Heart Association- Health Care Providers. If the CPR card that you have expires before December 16, 2012 you must retake it before the semester begins and present the copies of the card with the rest of the required documentation.
  • If you turn in a CPR card that doesn’t meet the above requirements you will be required to retake the course.

STEP SIX: Complete Clinical Modules

All incoming students must complete all of the following modules and post test after July 1st.

Print the confirmation page (last page) and submit with your completed packet.

The modules can be found on the Educational Resources page.

  • Fall 2012 Student Handbook - READ the Handbook and then print and sign the last page, Statement of Understanding- (will be posted by July 1st).
  • St. David's Mandatory Education Module
  • St. David's Statement of Responsibility
  • St. David's Confidentiality & Security Form
  • Seton Education Module
  • Seton Required Competencies reviewed and form signed
  • HIPAA Training Module w/ Confidentiality Form (2 parts: Submission page and Confidentiality Form)
  • Licensure Eligibility - BON Requirement

***If you do not have a copy of the last page then you will have to redo the module you are missing. Make sure you are doing your modules with a printer for your printed confirmations.

STEP SEVEN: ACC Email

STEP EIGHT: Turn in your completed packet between July 15th and August 1st (ONLY) at the Eastview Campus, Rm. 8227 for Austin Students. Student accepted to the Fredericksburg location will submit all paperwork to Cindy in the Fredericksburg Nursing Office.

The following items must be in your packet to be considered complete; incomplete packets will NOT be accepted.

  • Copy of front and back of AHA-BLS CPR Card
  • Signed and dated Health Data Physical/Tb Form- copy of Chest X-Ray if applicable.

*NOTE- Please make yourself a copy of all submitted items before you turn in your packet. We DO NOT make copies.

Once your completed packet is turned in you will receive a “Clinical Clearance Ticket” for the fall semester. This ticket will be given to your instructor and it is your confirmation that you are complete and clear for clinical for the spring semester.

STEP NINE: Register for the following courses during the registration period:

  • RNSG 1140 Nursing Skills for Articulating Students
  • RNSG 1170 Nursing Process for Articulating Students
  • RNSG 1263 Nursing Practice I-Clinical
  • RNSG 1517 Concepts of Nursing Practice I for Articulating Students
