Website Password Request Form
Basic password protection is not a strong form of data security.
It is adequate to protect information of a moderately sensitive nature from public view. Contact the IT department to help
with data security if you need to secure confidential information such as student or personnel records.
The help desk can't look up or reset the login information, but they can refer you to a system administrator for assistance if you forget
the password.
Use this form to request that a password be set for your website or part of your website.
Note: When your password is installed you will be asked for your username and password the next time you try to go to the password protected website. Enter the username and password you typed on this
request form to access your website.
