No new AFLAC policies are available. Only current employees with AFLAC policies may continue payroll deductions.

November is the month to cancel existing Aflac policies that you pay for by payroll deduction. Please complete this form and fax to HR Benefits at 223-7907 or 223-7625 by noon on November 30th to cancel your policy and stop payroll deduction. All cancellations will be effective December 1st. If you do not cancel your policy during November, you will have to wait until next year or when you experience a qualifying life event to make the cancellation.
