Annual Placement Process

All completed forms due to HR Compensation by June 30, 2008

Form 1 – Level Placement Update for Full-Time Faculty ( PDF)
Complete this form if you are full-time faculty and have received an advanced degree or completed additional graduate hours during FY08 that qualifies you for a higher salary placement. HR Compensation also updates overload status from this form.

Form 2 – Level Placement for Adjunct Faculty ( PDF)
Complete this form to report if you are adjunct faculty and have completed a PhD, MFA, or JD this year. This form is used for initial placement and for the annual placement update.

Form 3 – Related Work Experience: Initial Step Placement for Adjunct Faculty and Full-Time Faculty Overloads (Workforce and Health Sciences Only) ( PDF)
Complete this form if you work in Workforce or Health Sciences to report related work history that supplements the application for employment found in your personnel file.

Form 4 – Teaching Experience: Initial Step Placement for Adjunct Faculty and Full-Time Faculty Overloads( PDF)
Complete this form to list all previous college-level teaching experience for which you were the instructor of record.
