Form 1 – Level Placement Update for Full-Time Faculty (PDF PDF)
Complete this form if you are full-time faculty and have received an advanced degree or completed additional graduate hours during FY08 that qualifies you for a higher salary placement. HR Compensation also updates overload status from this form.

Form 2 – Level Placement for Adjunct Faculty (PDF PDF)
Complete this form to report if you are adjunct faculty and have completed a PhD, MFA, or JD this year. This form is used for initial placement and for the annual placement update.

Form 3 – Related Work Experience: Initial Step Placement for Adjunct Faculty and Full-Time Faculty Overloads (Workforce and Health Sciences Only) (PDF PDF)
Complete this form if you work in Workforce or Health Sciences to report related work history that supplements the application for employment found in your personnel file.

Form 4 – Teaching Experience: Initial Step Placement for Adjunct Faculty and Full-Time Faculty Overloads(PDF PDF)
Complete this form to list all previous college-level teaching experience for which you were the instructor of record.
