Accidents and Injuries

If an employee has a work-related accident, the employee should report the accident to the immediate supervisor or a member of management as soon as possible. The immediate supervisor is responsible for completing the Supervisor’s Injury/Illness Analysis and Prevention Report form located in the ACC Forms Inventory and coordinating the investigation process with Environmental Health, Safety and Insurance staff.
