INFORMATION FOR ALL FACULTY REGARDING INPUT FORMS
Use the links to the left to find the information that you are looking for. You can find the standards established by the Government Department Task Force.
More importantly, you will find the Faculty Input Form itself. You can download it in Microsoft Word, fill it in by typing your responses, save a copy and print one out to send to the Government Department Chair by the Deadline.
In addition, you will also find the evaluation instrument that the committee members will be using to evaluate your Faculty Input Form. You'll note that there are three general categories: student comments, statistical data, and instructional goals. Be sure to hit all three areas in your response. If you would like to see an example of a response you may also find that here, too.
Please remember that a Faculty Input Form must be submitted each February in order to remain on the eligibility list to teach.
