FACULTY INPUT FORMS
In non-portfolio years, the overall rating of one's input form is what will be checked the evaluation summary form. This is the form that faculty members receive from the Department Chair and the Dean.

Thoroughly and thoughtfully responds to student comments included in student evaluations.
Sets specific goals for instructional gains in the coming year based on student evaluations both statistical and narrative

Very Good
