Unit 15 Objectives: Quality Assurance in the Blood Bank

  1. State the goals of Quality Assurance (QA).
  2. Define the following terms:

    • quality control
    • quality assurance
    • continuous quality improvement
    • utilization review

  3. Define the elements of record keeping.
  4. List the elements of a technical procedure manual according to the NCCLS guidelines.
  5. List the reagents which must be tested each day of use.
  6. List the tests which are Quality Controlled (QC’d) with each run.
  7. State the information which must be documented during the reagent log in process.
  8. State the procedures used for monitoring temperatures.
  9. State the procedure utilized to document component quality.
  10. Describe the components of a job description.
  11. Describe how the competency of personnel is monitored.
  12. Describe the 10 step JCAHO process for quality assessment.
  13. Describe the function of the transfusion committee.
  14. State the purpose of the Quality Assessment plan.
  15. Define the purpose of determining the crossmatch:transfusion ratio.
  16. State other transfusion related situations that are utilized.
  17. List the audit criteria utilized to justify transfusions.
