CLINICAL PAPERWORK
You will need a one inch binder. Some students like to use tabs to keep each weeks paperwork so they know what to turn in each week. During clinical orientation I will tell you which forms will be turned in each week. Unless otherwise indicated print out ONE copy of each of the following:
- Accident Report Forms This will be provided by your instructor.
- Clinical Check Off Sheets
- Venipuncture Check Off Form - Revised Fall 2011
- Capillary Puncture Finger Stick - Revised Fall 2011
- Capillary Puncture Heel Stick - Revised Fall 2011
- Weekly Evaluation - one for each week EXCEPT the last week Revised Fall 2011
- Clinical Log Sheet -one for each week.
- Final Evaluation - PDF
- Venipuncture Clinical Competency Assessment Form Revised Fall 2011
- Capillary Puncture Capillary Form - Will be used if capillary punctures are performed.
SITE SURVEY
You are required to evaluate the quality of the experiences at your site. This allows us to make decisions about the future use of this site. PLEASE be objective and in your comments write your observations although it may also be appropriate to add how the activities and interactions affected you personally.
Although we ask for your name, rest assured that information will not show up on the compiled report. The anonymity is important so that you will be honest in your evaluation and comments regarding your clinical site.
