- ALL items must be checked off as completed.
- Take the completed form and your clinical paperwork to any instructor listed on the last page during their stated office hours.
- If you cannot come during the stated office hours email one of the instructor's to make an appointment.
- Incomplete packets will not be accepted. You will be required to return to the clinical site to have the documents filled out.
