Phlebotomy Handbook: Blood Specimen Collection from Basic to Advanced, 8th Ed. Becan-McBride, Kathleen. Pearson. (ISBN 0-13-513424-2).

    Print the following:

  • Clinical Tracking Assignments Form Use this form to aid you in making sure all necessary documents and modules get completed / submitted prior to your phlebotomy clinical rotation assignment. Failure to submit and complete all assignments on this form will delay your clinical rotation.
  • "Student Form to Create Clinical Schedule" This form is to gather necessary information that will be used to create your clinical rotation. The completed form must be submitted to your professor by the second class day. Failure to provide accurate information or submit this form on time will delay or prevent development of your clinical rotation.
  • Venipuncture Practice Skills Assessment Form Print TWO copies of this form for each lab in which a venipuncture will be performed.
  • Capillary Puncture Practice Skills Assessment Print out ONE copy of this form for each lab in which a capillary puncture is performed.
