To complete this form, you must answer the following questions. (Short answers are fine.)

The answers to all of these questions can be found somewhere on this website. If you don't know the answer to something, you need to go read through this website until you find that answer. (Click here to open another browser window to the website. That way, you can keep the form open and come back to it as you find the answers.) You may not just say "don't know." I promise you, all of this information is available on my website.

Do you know your TSI (this is the new name for the TASP) status, and if not, whom should you contact immediately?

Have you taken the "prerequisite review" pretest?
Yes No

If you answered "No" to the previous question, then STOP and go here right now and take the "prerequisite review" pretest. (This is a requirement.) Then come back and finish this form.

How many questions did you get right on the "prerequisite review" pretest when you went back and checked your answers? (You may not answer "can't remember" here; go back and look or take it again if necessary. This is important.)

What is the name of the textbook for this course?

Once you have opened the cellophane covering the textbook, will you be able to return the book to the bookstore?
Yes No

Where can you find the validation number that you will need to enter in order to continue using the software beyond the first two weeks of the course? (Warning: This means you need to be sure to not throw this away accidentally. Also, if you don't have this number, you need to conact the Bookstore right away.)

Where could you go to receive free tutoring for this course?

What toll-free telephone number can you call for free technical support on weekdays, from 10 am to 10 pm?

What is the deadline date for taking the final exam?

List four ways you can earn "class participation" credit.

Select the ACC Testing Center in which you will take ALL of your exams for this course:

Warning: It is your responsibility to be aware of the Testing Center hours (go here for info) and to arrange your testing around them. In particular, the Testing Centers at the San Marcos Center, Fredericksburg, and Round Rock have very limited hours. If you choose one of those Testing Centers, you must take your Final Exam on the first possible day they are open during the exam period. (Personally, I would avoid those Testing Centers because of this.)
