Fill out this form before you buy your textbook and course materials.

(Do this before the beginning of the semester; if you add the course after classes begin, you must submit it no later than one day after you register.)

ACC has recently started supplying all students with their own email accounts, which you are expected to use and the college will use to communicate with you. It is therefore really important that you check your ACC email address regularly. If you haven't already gotten your ACC email address and login information, you need to go here now for details on how to do that. (This is REQUIRED, so go do it now, please...) This is the email address I will respond to and send your course information to. Also, please double-check that you have typed it in correctly.

Have you taken the "prerequisite review for the pretest"?

If you answered "No" to the previous question, then STOP and go here right now and work the "prerequisite review for the pretest". (This is a requirement.) Then come back and finish this form. This will also help you study for the Elementary Algebra Pretest you need to take first thing, once you get started working in the software.

How many questions did you get right on the "prerequisite review for the pretest" when you went back and checked your answers? (You may not answer "can't remember"; go back and look or take it again if necessary. This is important.)

Select the ACC Testing Center in which you will take ALL of your exams for this course:


Warning: It is your responsibility to be aware of the Testing Center hours (go here for info) and to arrange your testing around them. In particular, the Testing Centers at the San Marcos Center and Fredericksburg have very limited hours. If you choose one of those Testing Centers, you must take your Final Exam on the first possible day they are open during the exam period. (Personally, I would avoid those Testing Centers because of this, or make arrangements to come into Austin for the final exam at least, if possible.)
