
1. Complete the automated form below, or complete a paper version of the form found by the coffee maker in the CE Department. Submit the automated form online, or place your paper version in the suggestion box by the coffee maker.
2. Your concern/comment will be received by the Chair of the CE Open Communications Committee, who will take your concern to the CE Leadership Team (Dean and Directors), during their regular Leadership Meetings on Monday.
3. Every attempt will be made to respond to your comments/concerns within one business week from the date it is received.
4. Your comment/concern will be posted (if you designate it to be posted) at the time the solution/response is posted, and not before that time. If you want the answer/response to be communicated to you directly, then you must include either your name, email or phone number ... and the CE Open Communications Chair will contact you.
5. The source for the answer/response will be posted with each answer/response.
