Prerequisite Check
In accordance with ACC policy, you must have the prerequisite in order to attend this class. The prerequisite (roughly) consists of (documented proof of) at least one of the following:
(1) MATD 0390 Intermediate Algebra or equivalent (C or better)
(2) A score on an assessment test as follows: THEA (270+), SAT (570+), or ACT (21+). In addition, you must have credit for second-year high school algebra (Algebra II).
(3) A score on a math placement test as follows: COMPASS (69+) or CPT (90+)
A more detailed description of the prerequisite can be found on my web page for the course.
Which of the above provide you with the prerequisite? (1}_____ (2) _____ (3) _____
For (2) or (3), state the test and qualifying score: _______________________
For (2), have you received credit for Algebra II? Yes____ No____
If you have any doubt about your prerequisite status, please see me or an advisor immediately, because… if you cannot prove that you have the prerequisite, you may be dropped from the course at any time!
Who advised you to take Math for Business and Economics?
advising office__ math. dept. advisor__ other (specify) _____________________________
Please note: If you completed your last math class more than two years ago, or you received lower than a B, or the Prerequisite Review shows that your basic algebra skills need sharpening, I recommend that you refresh your algebra skills by first taking (or re-taking) Intermediate Algebra for at least a B before embarking on Math for Business and Economics.
I have answered all questions truthfully, and have read the above advice. I understand that my instructor may drop me from this course if I do not have the proper prerequisite background. I understand that failure to give accurate information about prerequisites constitutes scholastic dishonesty.
