Proof of Prerequisite for ______________________________________

Note the following from the ACC Course Schedule:

Courses with course prerequisites have a plus (+) before the synonym.   You must provide proof of the prerequisite (grade slip, transcript, etc.) at the first class meeting.  Prerequisites are listed in the ACC Catalog.  Without proof, you will be dropped from the class.
The prerequisite for College Algebra is MATD 0390 with a C or better or satisfactory score on the ACC Mathematics Assessment Test taken before enrolling.  For detailed information, see the web site http://www.austincc.edu/mthdept2/advising/chart.htm

____  It is based on a C in Intermediate Algebra . Note: it is extremely rare for a student with a C in Intermediate Algebra to pass College Algebra.  Please retake Intermediate Algebra as many times as it takes to get an A or B.

This does not necessarily mean that you don’t have the prerequisite.  It means that I did not find the information in your ACC record because: it was not yet posted, Intermediate Algebra is from another institution, etc.  If you think you do have the prerequisite and wish to remain in the course you need to provide the appropriate documentary evidence, or take this form to Advising (room 1131, across from the cafeteria), have an advisor review your status, and return the completed form to me.  Without this proof, you will be dropped.

_____  You have qualifying TASP, SAT, or ACT scores, but you also need proof of C or better in Algebra II.  As a matter of fact, these tests do not offer accurate placement, anyway.  Go to Advising and ask to take the COMPASS test.
______You have borderline T-COM, CPT, or ASSET scores.  See Advising for “Decision Zone” verification.
