Please note that these forms are word documents although you access them from the IPR website.
You will need to save the form to your computer
before you complete it. If you type on the form
that is on the web, your information
will not be saved. Once the form has been completed, send the completed
form as an email attachment to IPR co-chair for your area (
workforce or
academic transfer). These instructions will walk you through the process of filling out the
form and emailing it.
Saving the form to your computer
Click on the following link to access the Instructional Program Review forms:
http://www.austincc.edu/progrevw/schedule.php.
Select the link for the appropriate form.
Using Internet Explorer:
- Click 'File' on the menu bar at the top of the screen.
- Click 'Save As'.
- Save to your computer as you would any document.
- Please name the file in the following format:
Your program name, underscore, form name.doc (Example: OIE_IPRS.doc).
- Please make a note of the location and file name under which you are saving this document. (Example: C:/My Documents/OIE_IPRS.doc).
Using Netscape:
- If prompted to open or save file, click save, otherwise follow the instructions above for Internet Explorer.
- Save to your computer as you would any document.
- Please name the file in the following format:
Your program name, underscore, form name.doc (Example: OIE_IPRS.doc).
- Please make a note of the location and file name under which you are saving this document.
(Example: C:/My Documents/OIE_IPRS.doc).
Filling out the form
- Type your responses into the highlighted boxes. The boxes will expand as you type.
- Hit the 'Tab' key or use your mouse to move from box to box.
- Type as you normally would, including using the 'Enter' key for paragraphs.
- Use your mouse to click on "check" boxes. An 'X' should appear in the clicked box.
Submitting the form
- Once you have completed the form, save it to your computer by selecting 'Save' or
'Save As' from the File menu on the toolbar, and save as you normally would a document.
- Open your email account and send as follows:
- Address email to the co-chair for your area (workforce or
academic transfer)
- In the Subject line type - IPR Form post to web
- In the body of email type - Attached please find the IPR Form for (program)
- Attach your saved file to the email
- Send email
Within a week, you should receive a confirmation that the forms have been received.
