		Instructional Program Review Summary Form (Word Document) - This report summarizes the findings of the self-study team, discusses the impact the findings have on the quality of program instruction, and concludes with recommendations for future program improvements.
		Quality Improvement Plan Form (Word Document) - This form presents the program's four-year plan, including resource allocation requests, for making improvements to the program. It is used to inform the College's planning and resource allocation processes.
		Program Status Form (Word Document) - The Instructional Program Review committee members use this form to evaluate the program based on its self study review.
