  6. Official High school or College Transcript and Diploma (translated into English)

  8. Recommended vaccinations are those listed on the Centers for Disease Control and Prevention website as well as bacterial meningitis. The bacterial meningitis vaccination is required and must be administered no later than 10 days before the first day of class.

Important: If you are on a J-1 visa, and you want to change to an F-1 visa, you have to check your DS-2019 and your J-1 visa to see if you have the Two-year Rule Restriction. If you do have the Two-year rule restriction, then you cannot apply for an F-1 visa since you have to return to your country for two years. However, if you do not have the Two-year Rule Restriction, then you are eligible to apply for an F-1 visa by submitting a change of status application (I-539) to INS through the ACC International Student Office. The Change-of-Status can be done only if your I-94 card has not expired and the application must be submitted to us at least one month before the expiration date on the I-94 card.

Please contact the ACC International Student Office for the Change-of-Status application and instructions on what you need to submit with the application.
