Telephone Service Request Procedures

Explanation of charges:

Subcode 6571: Installation of new phones or moving phones or for installing phone and computer cable drops. (one-time charge per service for labor and materials)

Subcode 6701: Cost of new phone instruments (one-time charge per instrument)

Subcode 6569: This is for monthly T-1/circuit charges. Fees will be billed up front for the entire fiscal year, therefore you must have appropriate funds in your account to cover all monthly charges. Please contact Telephone Services for install fees, monthly charges, terms, and agreements.

Subcode 6565: This is for monthly telephone charges ONLY. Do not send us a requisition using this subcode. You are automatically charged from this subcode every month for telephone charges $30/month for each remaining month of the fiscal year, plus estimated long distance usage for each phone. However, you should make sure if you are adding a new phone that there is enough money in this budget to cover charges.

PLEASE NOTE: As of 9/1/07, No Purchase Requisition is required for telecom work/service requests. Please continue to Step 3 to complete the online telephone service request form. All requests for new phones or cabling must still be approved by the Department Dean or Budget Authority. Thank you for your cooperation.


Enter Datatel PR - Go into Datatel and do a purchase requisition for the appropriate amount as follows:

Vendor: ACC Telecom (#5678)
Buyer: Jodie Scherer (#402292)
Subcode: see above
Description: provide brief description of the telephone service and/or equipment being requested, including room number
Amount: See Estimated Costs for more information.

Approve Datatel PR
The Datatel purchase requisition must be approved in Datatel before any work can begin.
