Beneficiary Designation

Don’t leave your loved ones with a financial mess! Designate your beneficiary for your life insurance and retirement accounts! So many accounts – who do I contact? It’s all right here!

When an ACC Employee dies

In the event of death, ACC no longer accepts beneficiary designation forms for an employee’s final paycheck. ACC will distribute the final paycheck to the legally authorized representative of the deceased employee. Contact the HR Benefits office if you need assistance.

Final ACC Paycheck Process

Review the following information about pay and benefits in the unfortunate event that an ACC employee passes away:

Life Insurance

The Employees Retirement System of Texas (ERS) administers insurance benefits for ACC employees, including life insurance. You must log in to your ERS OnLine account to designate your beneficiary. Once you designate your beneficiary, ERS will email a PDF of your beneficiary designation to the email address they have on file. To complete the designation process, print, sign, have witnessed, and return the designation form to ERS. Do not return this form to HR Benefits. You will receive a second email from ERS confirming the completion of your beneficiary designation. Log in to ERS OnLine.
