1095-C forms are posted in the ACC self-service portal. If you don’t receive your 1095-B form, contact your health plan carrier.

Health Plans

The medical plans offered by ACC are provided through the Employees Retirement System of Texas (ERS). Most employees have a choice of two comprehensive medical plans that cover preventive care, doctor visits, hospital care, and prescription drugs. Your ERS medical coverage also includes $5,000 of Basic Life insurance and Accidental Death & Dismemberment coverage. Dental Insurance

Employees may elect dental coverage for themselves or eligible benefits. See details on the Dental Insurance for Employees webpage.

Vision Insurance

ERS medical plans cover one vision screening per year for each participant. You may enroll in an optional vision benefit plan through ERS to receive benefits for eyewear, contacts, and other vision care needs.
